As we have eliminated the hassle of “Personal Information Use Agreement” paper forms and redirected the Student Health Center’s medical records and registrations online, please follow the instructions below prior to your visit to receive faster care. 

Log in to “Trinity” – Student Guide – Welfare, Housing – Student Health Center – Agree the Use of Personal Information and Save – Choose an appointment time, and illness type – Submit application
